What is Service Coordination?

Service Coordinators work alongside individuals to identify goals and objectives for optimal independence, functional capacity, and quality of life. Together, a participant and Service Coordinator create and develop a comprehensive service plan which acts as a roadmap to guide participants to their fullest potential. In addition, Service Coordinators act as a gateway to community resources for participants. They administer service options for participants to choose if, when, and where they wish. 

Service Coordinators also set up social and community-based transportation and collaborate with participants to make sure that they are satisfied with their services and transportation is scheduled as necessary. They also make recommendations and adjustments as necessary to the service plans.

What are the requirements for the NHTD Waiver?

To qualify for the NHTD Medicaid Waiver, you must be:

Capable of living in the community with needed assistance from available informal supports, non-Medicaid supports, or Medicaid State Plan services and require one or more waiver services

Eligible for the nursing home level of care Authorized to receive Medicaid Community Based Long Term Care

At least 18 with a physical disability or 64 years of age or older. Considered part of an aggregate group that can be cared for at less cost in the community than a similar group in a nursing home (elderly and/or disabled)

Choose to live in the community as a participant in this waiver rather than in a nursing home

Not participate in another HCBS waiver

What services are available?

Numerous services are available through the TBI and NHTD Waiver Programs. 

Environmental Modification: Provides adaptations to residences to promote independence and to ensure safety.

Community Integration Counseling: counseling is provided to participant and family members to assist with issues related to community integration. 

Independent Living Skills Training: provides one-to-one training to focus on increasing independence in functional skills such as cooking, shopping, banking, financial management, travel training, etc.

Structured Day Program: Community based social model program to provide socialization and skill building opportunities

Home and Community Support Staff: provides oversight, cognitive cuing and support to enable the waiver participant to reside safely within the home and community environments.

Substance Abuse Program: Services implemented based on the unique challenges Associated with TBI to reduce and/or eliminate substance abuse that interferes with community integration

Positive Behavioral Intervention and Support: Intensive Behavioral Program: Community based provision of direct technical assistance to individuals with challenging behaviors and staff training for development of program support strategies.

Assistive Technology: provides durable and non-durable equipment not funded under state plan Medicaid.

Respite: provides the primary caretaker a needed respite from providing day to day care to the participant.

What are the requirements for the TBI waiver?

To participate in these waivers you must:

TBI Waiver:

Be diagnosed with TBI or a related condition

Eligible for nursing facility level of care

Enrolled in the Medicaid program Between 18 and 64 years of age, and injured after the age of 18

Choose to live in the community rather than a nursing facility

Have or find a living arrangement which meets your individual needs

What is the cost of these services?

There is no cost. The TBI and NHTD Waiver Programs are paid by Medicaid.
